How to Request Academic Documents
Walk-In at the Office of the Registrar
Contact and payment hours: 8AM – 4PM
1. Fill in CR42 and/or CR42/1 and press queue number 4.
– If you would like to have the documents sent by postal service, also fill in CR42/2.
2. The officer will check your name, student number, identification or passport number, and your student status.
3. Pay the document fees and postal service fee (if applicable).
4. If you are picking your own documents, the documents will be available within 30 minutes. You will have to present your ORIGINAL identification card, student card, or passport to pick up the documents. You can pick up your documents later at your convenient time within 60 days.
* If there are any additional forms to be completed, certified, or sealed on signed envelope, the officer will make an appointment date to pick up the documents later.
Authorize a Third Party
Contact and payment hours: 8AM – 4PM
1. The owner of the document (the student or the graduate) complete the authorization form. Both parties should sign their name on the form.
2. The third party picking up the document must bring the authorization form and copies of both parties’ identification documents to the Office of the Registrar.
– If the third party is also requesting the documents and pay the fees on behalf of the student, proceed accordingly with the WALK-IN process. The authorization form and copies of the identification documents will be required to pick up the documents. The third party must present their ORIGINAL identification card (or student card, or passport).
– If the third party is picking up the documents that have already been requested and paid, or picking up the academic documents for recently graduated students, the third party should submit the authorization form and copies of the identification documents. The third party must present their ORIGINAL identification card (or student card, or passport).

Request Documents Online
Manual for Using Document Request System
If you are in urgent need of academic documents (CR23, CR24, CR25, CR26), it is recommended to walk-in to the Office of the Registrar to request the documents. They will be available for pick-up within 30 minutes. Contact and payment hours: 8AM – 4PM.
1. Complete the Request Form online
– Input your student number and identification number (passport number)
– Input the 4-digit CAPTCHA and click “New”
– Choose your preferred documents. Click “Save” and check the requested information
– Click “Print” to confirm your request. You will be directed to the page with list of documents and total amount of fees to be paid.
2. You can pay via generated QR Code or by the Bill Payment Pay-in Slip.
3. After your payment has been confirmed, your documents will be printed and sent.
– Check mailing service fees.
