Students should log in using Chulalongkorn Student e-mail address (6xxxxxxxxx@student.chula.ac.th) to fill the form. Send the filled form in PDF format along with supplementary documents (if applicable) to saraban_reg@chula.ac.th

- Request Form for Student ID Card (CR50)
Instructions (for forms to be submitted to the Office of the Registrar)
- After filling in all information through Google Form, the PDF file of the form you filled will be sent to your student e-mail address.
- Check the correctness of the filled information, sign your name.
- Submit the PDF file along with supplementary documents (if applicable) to saraban_reg@chula.ac.th
- The Office of the Registrar will process your request.
How to Access Student E-Mail Address and Retrieve PDF File
- You can access your student’s e-mail address through Student’s E-mail page or at https://outlook.com/chula.ac.th
- The PDF file that was sent should be in the Inbox folder and the sender is curegform@team.chula.ac.th
- If the PDF file is not available in the Inbox, please check the Junk Email folder.
- If there are any inquiries regarding the request form, please send an e-mail to webreg@chula.ac.th with your student ID number and the reference number of the document as appeared on the e-mail subject.

Authorization Forms (to pick up academic documents)

- After filling in all information through Google Form, the PDF file of the form you filled will be sent to your e-mail address.
- Check the correctness of the filled information.
- Both parties should sign their names on the form. Bring the authorization form along with the copies of the authorizing person and the authorized person to the Office of the Registrar to pick up the academic documents.
