Request for Major/Field of Study Change

If students would like to change their major or field of study, pleases proceed as follows:

1. Complete Request Form To Select or Change Field of Study (CR5)
2. After submitting the form, you will receive a CR5 form in PDF format via your student e-mail (66xxxxxxxx@student.chula.ac.th).
3. Submit the form to your Faculty’s Registrar Office.
4. After approval, check your information at the website. Log-in and choose “Personal Information” -> “Student Personal Records”.
