Students are obliged to check the correctness of title, name and surname. Modification can only be done during period of study. After graduation, any changes are not allowed.

To change or correct your title, name, or surname, please proceed as follows:

Walk-In at Office of the Registrar

  1. Fill in CR47 form. Submit the form along with the following documents:
    • Thai Students
      • changing/correcting Thai name: a copy of identification card, a copy of certificate of name change
      • correcting English name: a copy of identification card
      • changing title: an order indicating title or rank change, or a certificate of marriage, whichever is applicable
    • International Students
      • changing/correcting English name: a copy of passport, a copy of certificate of name change (if applicable)
  2. To request for a new student card after name change, a 100 baht fee will be charged.
  3. (If a new student card is also requested) Three working days after submitting the form to the Office of the Registrar, present the following documents at Kasikorn Bank (Chamchuri Square Branch, Siam Square Branch, Chulalongkorn Hospital Branch, MBK Center Branch, or Samyan Mitrtown Branch) to retrieve your card during the bank’s business hours.
    • Receipt Form for a Student Card (issued by the Office of the Registrar)
    • Original identification card (Thai students) or passport (foreign students)
    • Kasikorn Bank passbook (In the case of an e-Savings account, K PLUS application can be used instead of a passbook).
    • An original copy of certificate of name change (if applicable)

Request Online via www.reg.chula.ac.th

  1. Complete Request Form for Title, Name, Surname Change (CR47). If you would like to make a new student card, transfer the fee of 100 Baht as specified in the form (please read the transfer details in the form and follow all instructions carefully).
  2. After submitting the form, you will receive a CR47 form in PDF format via your student e-mail (6xxxxxxxxx@student.chula.ac.th).
  3. Send the PDF form together with supplementary documents (No.1 of “Walk-In at Office of the Registrar”) and proof of payment (if requesting new student card) to saraban_reg@chula.ac.th
  4. (If a new student card is also requested) Once the Office of the Registrar has verified all information, a document for issuing the student card will be sent to your student email within 3 working days. Present the following documents at Kasikorn Bank (Chamchuri Square Branch, Siam Square Branch, Chulalongkorn Hospital Branch, MBK Center Branch, or Samyan Mitrtown Branch) to retrieve your card.
    • Receipt Form for a Student Card (issued by the Office of the Registrar)
    • Original identification card (Thai students) or passport (foreign students)
    • Kasikorn Bank passbook (In the case of an e-Savings account, K PLUS application can be used instead of a passbook).
