There are two types of Sick Leave:
- Sick Leave Prior to Examinations if students fall sick before the study period of that semester ends, remain sick until the examination date, and are unable to take the exam for one or more courses, students should take sick leave prior to the examination (the period to take leave of absence has already passed).
- Sick Leave During Examinations if students have completed their studies but fall sick during the examination period and are unable to take the exam for one or more courses, students should take sick leave during examinations.
Sick Leave Procedure
- Complete Request Form for Sick Leave (CR44)
- After submitting the form, you will receive a CR44 form in PDF format via your student e-mail (66xxxxxxxx@student.chula.ac.th). Submit the CR44 form (with an agreement from the advisor) to the Faculty’s Registrar office along with the doctor’s certificate issued by a government hospital, a private hospital accredited by the Ministry of Health. The doctor’s comment on the student’s sickness must be dated on the same date and time the student is absent from the examination.
- Contact the Faculty’s Registrar office or the Office of the Registrar for result.
- CR44 must be submitted within 1 week after the date of the sickness. You can also refer to No.13.2 of Regulations on Undergraduate Studies B.E. 2556 or No.53 of Regulations B.E. 2565, Announcement on Grading Criteria for Students on Sick Leave B.E. 2557, No.113 of Regulations on Graduate Studies B.E. 2551 or No. 85 of Regulations on Graduate Studies B.E. 2561
- If the sick leave request is approved, the highest grade symbol awarded for the course that the students have missed the exam are tentatively as follows:
- A grade not higher than C (Undergraduate Students) according to No.3 of Announcement on Grading Criteria for Students on Sick Leave B.E. 2557
- A grade not higher than B (Graduate Students) according to No. 84 of Regulations on Graduate Studies B.E. 2561
- An achieved grade according to No.4 of Announcement on Grading Criteria for Students on Sick Leave B.E. 2557
- Students whose sick leave have been approved and received I (Incomplete) in their grade report must ensure to have that I changed, otherwise that I symbol will be changed into F or U within the first two weeks of the following semester.
